Background: Transmission to and use of electrical energy for households or railways leads to the emission of extremely low frequency (ELF) magnetic and electric fields. Adverse health effects due to ELF magnetic field exposure at intensities below legal threshold values are a matter of current debate. For some neurodegenerative diseases like Alzheimers disease or amyotrophic lateral sclerosis (ALS), there is increasing evidence for an association with occupational magnetic field exposure. This study will examine whether long-term exposure to ELF magnetic fields from power lines is associated with mortality from neurodegenerative diseases, in particular ALS. Methods: The project will be nested in the Swiss National Cohort (SNC) study. In the SNC, mortality data are linked to census data of 1990 and 2000, which include the geo-coded residence of each person as well participants' characteristics, such as socio-demographic variables, occupation or characteristics of place of residence. Analyses will be based on approximately 7 million individuals, 70 million person-years of follow-up, 1500 deaths from ALS and 17000 deaths from Alzheimers disease. Individual electromagnetic field exposure will be predicted based on a model allowing for the distance between place of residence and power lines and line characteristics. The exposure model will be validated with a measurement campaign. The potential association between neurodegenerative disease mortality and exposure will be evaluated by means of Cox proportional hazards models. Significance: This will be one of the largest studies ever done to address the potential association between neurodegenerative diseases and exposure to electromagnetic fields from power lines. If confirmed, such an association would be of high public health relevance.
